Check the shell for a small hole near the foot. If it has one then somehow a predatory Pyramidellidae snail got in your tank. It could be lots of things, check out this page to see if you can narrow it down.

Just as Jeff said,my clam was fine for years,I picked up a zoo colony dipped it and still got a snail from it.
