I don't quarantine so sometimes new fish will come in with ich after a few days. But I never lost a fish because of it. The ich is gone after a few days. So I never worry about it. Fish get healthy then ich is gone. That just my two cents. Of course all my fish are common and cheaper ones.

I removed sand a few months ago and totally love it. The bottom is now totally covered with coraline and the Kole Tang keep it free of algae/diatom. It's personal preference aesthetically but it's definitely easier to maintain. Only thing is not able to keep some of the sand dwelling fish.
When I cured rocks I used Brightwell PhosphatE. I know it is not a long term solution but after two applications the rocks were no longer leaching PO4.
