Before you do anything I would have your water tested by a local reefer or LFS. Could be a bad test kit in the first place. Also whats your SG? I'd be more inclined to think with the heat and evaporation while you were away that could have caused your issue in the first place.
My vote is RTN. If it was the angel or blenny it would be missing parts of the flesh. Looking at the pic, half of the flesh has receded. Either cut the good part and put it on a new plug or put CA glue around where the tissue is receding. I would also have some one else test your water to verify...
People like this are the reason activist groups keep trying to push legislation to stop the the import/export of fish and coral.

Is this all of a sudden? Or has it been progressively getting worse? How's the flow in your tank? Any fish bothering it? I've had clowns in the past that would try and host them and would slowly kill them off.

The main reason I feel people dose vodka,vitamin c, or any other carbon source is to help lower the NO3 and Phosphate. Right now you have the opposite going on. I'm not saying dump a whole can of food in but maybe an extra feeding per week of reef roids, rods food, or what ever your favorite...

For the LED shadowing I would try physically raising your lights. That should give you better spread but you may need to increase the light intensity. Another option is change the optics on your fixtures. There's a huge difference between 60 and 120 degree optics in terms of penetration...
