Once the worm gets out of its tube it is pretty much over. Losing the crown is not a big deal, but losing the worm is. If it is connected to other feather dusters, you probably should remove it.
Phosban or chemipure works. The hair algae in your tank sucks up all the nitrates and phophate in your tank so your tests will come back 0 even thought there is plenty in your water. How long do you run your light during the day? Also, what seems to help is making sure you blow all the old food...
I lost a bubble tip this week as well, my sympathies. I did a 10% water change that day, and an additional 10% the next day as I was actually syphoning off pieces of my bubble tip from the sand bed after it cooked itself- you may have the same problem if you had 75% decentigration.

If you know someone who has live sand, you can always ask for some extra to seed your tank. You don't need a full live sand bed to start a tank, just seed from an established tank. Personally, I went to Tropiquarium and bought the live sand there at least they have several different sized bags...
