Have you cleaned the "venturi" parts. If you are not getting an increase in velocity in the venturi part of the skimmer, you will not get a good draw of air into the unit.

We used a homemade durso in our HOB overflow. Without it, it sounded like a toilet flushing. Just use a pipe that loosely fits in the drain hole. Here's two pics:

If they are feather dusters, not a problem. If it's aptasia, that's a problem.
Do you have a picture or could you google feather dusters and aptasia and see if your "worms" look like either of these two?
One way is to do it the same way they do it at the shore.
Place a net in the tank and leave it for awhile...let the crab get use to it being there. Then tie some tasty treat to a piece of string an go crabbin'. Dangle that treat close to the net and wait....and wait...this is best done...
