Nagel (now there's a name from the past) used to keep black paper on the sides of the tank walls to prevent light from approaching the tank from the side. This apparently increased survivors. That's about all I remember, but if you search for nagels threads here you may find something worth reading.

By the sounds of it, all exposed fish need to be culled, regardless of being symptomatic. That's pretty terrible. Should an asymptomatic fish be sent in for analysis (maybe 2 or 3)? I wonder if that would yeild better results?

If you go through the process of irradicating ich from your system, you'll need to make sure to TREAT (not just QT) each and every fish that you later want to add to your tank in order to maintain an ich free system. While most fish are asymptomatic, I'd venture to guess 90% of them are...

Sell them to locals or fish stores! Reduce our dependancy on wild caught! :)
