The YT Zebrasoma flavescens is a fantastic fish - but it is a big time eater and pooper. Good luck keeping it in that sized tank. Assuming you don't give it away, it will be good incentive for you to go get a big tank set up! ;)
well I may be showing my preference in terms of video games (RPG) - but it looks kinda like a malboro.

I was thinking that given all the bacteria and what not in a tank - if the fish's fins are being bitten/nipped at - then it could get infected, or potentially stress the fish out thereby causing a negative immune response...which promptly results in ich/velvet...
I would take the injured fish...

I'm not up to speed on all the common names- but dendro - is that related to a sun coral Tubastrea aurea? It looks similar.
Also - in the foreground to the left - did you purchase that anemone or was it hitching on the frag? It looks like a "flower" anemone...but not sure.
Personally of the damsels I always thought the Yellow-tail was nicer (chromis and clowns excluded). If you take away the Yellow-tails LR/hiding place home / - and re-arrange everything it will mellow out (or should). Probably the most vicious damsels in my opinion are the blue devil and that...

I assume you have an overflow?
I'd try and put two on on end, then the other plus the return of the overflow on the other side...So then the center of the tank would get flow from both sides. Experiment and see how that goes.
they are noisy and are "safe" in the sense they won't eat your reef. If a small fish or invert is dumb enough to "walk" into the pistol - it may stun / kill it.

what kind of powerheads/flow (gph) do you have in the tank? Do you have a pic of the tank with the cloudiness?
It's rather unusual for a tank that is mature to have any cloudiness from the substrate. Did you add sand or a new ph? just thinking out loud what might be causing this.

personally I would not re-use (ALL) the sand from an established tank. I'd start a brand new bed of sand and take a cup of my old sand from my old tank to re-seed the new tank.
