The starfish is an asternia species. Some feed on corals others are algae grazers. I have them in all my tanks and never had any issues with them. Others will tell you otherwise.

Hopefully the antibiotics will stop an infection. You can always apply neosporine to the wound a couple times a day to help it heal a little better. Of course that requires catching him to do it.
